I'd like to request disclosure for family court proceedings about myself
Thank you. Just follow these steps to apply.
Step 1: Download the disclosure of information in family court proceedings document and read the guidance notes.
Step 2: Print and complete the 'Standard request form for disclosure of police information' (you'll find it in Annex D) and post it to us along with your cheque – find out about the fees for this request and how to pay.
